For parents new to MYART registration day can seem a bit hectic. Typically a lot will be going on. Costume construction may have started. Children being registered and their siblings will be playing nearby. Rehearsals will be under way. Things can get a bit noisy. A lot of parents will be present. Many of them experienced MYART parents who will be happy to share their knowledge and experience. Others will be new to MYART as well.
Rehearsals start immediately. Parents who have not filled out registration paperwork may have their child attend rehearsal while they do so.
Parents should bring a pen, find the line for the registration table and be prepared to fill out the forms listed below. Once parents have finished filling out registration forms they may attend rehearsal. The first three rehearsals are open and parents may attend. Rehearsals are closed beginning the fourth rehearsal. Once rehearsals have closed parents may no longer attend.
This is a fairly typical registration form. Among other things, it requires name, age, address, phone numbers and email address. Please fill out the front side only. The back side is for office use.
This form is required by law. MYART must have this form at all rehearsals. Information to fill out includes name, address, phone number, doctors name and phone number, insurance information, hospital preference as well as alternate contacts.
Photographs are taken during rehearsals and during performances. These photos may be used for promotional purposes and on the website. Children are never identified in photos. This form grants MYART permission to use photos which include your child.
This form is for ordering a script, rehearsal audio tape and a show t-shirt. These items can be purchased individually or as a package at a reduced price.
MYART company t-shirts are black and have the MYART name and logo printed on them. To order a company t-shirt use this form. Please note that the show t-shirt mentioned above is not the same as a company t-shirt.
Payment plan forms are provided upon request. Please ask for one to setup monthly payments for the registration fee.
